Control Program

Jackson Hinds Comprehensive Health Center is excited to announce its Hypertension Control Program, a part of the National Hypertension Control Initiative (NHCI). The program is instituted by the Health Resources and Services Administration (HRSA) and the Office of Minority Health (OMH).

A key element of the program is remote patient monitoring (RPM), in which patients’ blood pressures will be monitored outside of the clinical setting.

  • Patients will transmit the readings through an application compatible with our electronic health record (EHR).

  • A member of the Hypertension Control Team will monitor blood pressure readings daily and provide appropriate instructions.
